Are you familiar with gout, a condition where even the slightest touch can result in excruciating pain? It's characterized by redness and swelling, often at the base of the big toe, accompanied by throbbing pain even without any obvious injury. Here, we'll explore the topic of uric acid, the primary cause of gout, explaining it using straightforward language. We'll also examine strategies for preventing hyperuricemia and techniques for handling gout attacks should they arise. Lastly, we'll touch upon a condition with a similar name called pseudogout.
Firstly, what exactly is uric acid? The name originates from its discovery in urine. Our bodies, starting from a single cell, shape the human form through timely cell division. Each cell contains genetic information, stored in DNA or RNA, collectively known as nucleic acids. These nucleic acids contain purines, which, when broken down, result in uric acid. Our bodies constantly undergo metabolism, breaking down old cells and producing uric acid from the breakdown of DNA and RNA. This uric acid is then excreted in urine by the kidneys. Essentially, uric acid is one of the substances produced daily in our bodies as part of normal metabolic activities.
The amount of uric acid in the body is tightly regulated to remain constant. Purines, found within cells, are obtained from both animal and plant foods. They are essential for the healthy functioning of our cells, necessitating their appropriate consumption through daily meals. Purines in food are primarily broken down in the liver into uric acid, which is then excreted from the body. On average, around 700mg of uric acid is produced daily in the body and excreted to maintain a constant level (approximately 1,200mg for healthy adult males), known as the "uric acid pool."
Uric acid can dissolve in water and is efficiently excreted in urine and feces. However, when there's an excess of uric acid, it can crystallize. These crystals, shaped like sharp spears, gradually form in various parts of the body when the uric acid level in the blood exceeds the solubility limit of 7.0 mg per deciliter. Typically, they accumulate at the base of the big toe, causing redness, swelling, and intense pain—a condition known as gout. Persistent hyperuricemia can lead to complications such as urinary stones and renal failure, highlighting the importance of prompt medical attention.
Even after the acute pain of a gout attack subsides, it may recur as long as uric acid crystals remain in the joints. Therefore, efforts should be made to prevent recurrence after properly relieving the acute pain. Colchicine has long been used as an effective treatment for gout attacks. Additionally, non-steroidal anti-inflammatory drugs are commonly used orally or topically. To prevent recurrence, it's essential to lower the uric acid level to below 6.0 mg/dL. Lifestyle changes, such as hydration, limited alcohol consumption, calorie monitoring, and regular exercise, are also crucial for prevention.
Finally, let's explore pseudogout—a condition with a name similar to gout but distinct characteristics. In pseudogout, although there's no increase in uric acid levels, crystals of calcium pyrophosphate are deposited in the joints, causing intense pain similar to gout. Unlike gout, which often affects small joints like the base of the big toe, pseudogout typically affects larger joints such as the knee. While there's no specific effective treatment for pseudogout, pain control is achieved using anti-inflammatory drugs.
In conclusion, gout, with its agonizing pain even from the slightest breeze, underscores the importance of understanding uric acid and adopting preventive measures. By paying attention to our daily eating habits and exercise routines, we can strive for better health and well-being.
